Other: Electrical outlets are inaccessible, there are no recalled and or prohibited toys or sleep/play equipment observed on the premises. Window cords were inaccessible to children.
Restrooms: The classroom has one diaper changing area. According to the facility representative all children are in diapers and are not being potty trained. The Center has a staff bathroom.
Outdoor: Outdoor play equipment was inspected for health, safety, good repair and age appropriateness.
LPA observed the infant outdoor space had one portable climber in the space. LPA did not observe additional items in the outdoor area for the infants to enjoy. LPA observed the padded mats that cover the cement were scattered around the space and overturned. LPA observed approximately ten sharp rocks and a piece of cement with chain link fence attached in the infant play space. Facility representative stated the public gained access to the space over the weekend and ripped up the pads. Per facility representative, the staff have not had time to organize the outdoor play space yet. Per facility representative due to the tropical storm earlier this week and the hot summer weather the infants have not played in the outdoor space. A technical advisory notice was provided to the facility.
Posting Requirements: Center has required posting posted (license, PUB 393, car seat law information).
Due to time constraints LPA was unable to complete the entire annual inspection. LPA will return to the Center on a future date to complete a review of the staff and child files.
Based on LPAs observations on this day, two type A citations, three type B citations and advisory notices were issued.
LPA Zirbes informed facility representative Zarary Ochoa that this report dated 8/23/23 documents two Type A citations which shall be posted for 30 consecutive days as there are immediate risks to the health, safety, or personal rights of children in care.
Also, LPA Zirbes informed the facility representative Zarary Ochoa to provide a copy of this licensing report dated 8/23/23 that documents any Type A citations to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representative Zaray Ochoa.
