25 | On September 11, 2023, at 09:30 a.m., Licensing Program Analyst (LPA) Kendal Zirbes conducted an unannounced Plan of Correction (POC) inspection and met with facility representative Zaray Ochoa. LPA disclosed the purpose of the inspection and was granted entry into the facility by the Facility Representative. Five infants were present with one teacher and one aide, and additional administrative staff members.
During this inspection, LPA and facility representative Zaray Ochoa completed a tour of the facility at approximately 09:35 a.m.. LPA observed the following:
1. The outdoor play space for the infants was free of hazards and was organized.
2. There was a crib for each infant in care
3. One infant was observed engaged in tummy time with age appropriate toys, another infant was laying on their back with a rattle and placed under a interactive toy. Teacher was observed engaging with the infants and toddlers.
4. Age appropriate toys were purchased and present in the indoor activity area.
5. Needs and Service plans have been updated to include a feeding plan.
6. Immunization have been obtained.
7. Attendance records were signed by authorized representative.
8. Facility was within the licensed capacity.
Based on LPAs record review and observations, the citations issued on August 23, 2023 and August 24, 2023 have been corrected.
A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Facility Representative Zarary Ochoa.
