25 | On October 9, 2023, at 1:00 p.m., Licensing Program Analyst (LPA) Kendal Zirbes met with Facility Representative Zaray Ochoa. The purpose of the inspection was to conduct a case management - annual continuation inspection at the licensed facility. LPA disclosed the purpose of the inspection to the facility represemtative. Present during today’s inspection were 9 school age children, one teacher, and additional staff members.
During the inspection, LPA reviewed five school age files and five personnel files. Based on LPA review, the school age files were complete. One of five personnel files were missing the health screening. An advisory notice was issued.
Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Based on LPA observations, and record review, one advisory notice was issued.
A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Facility Representative Zaray Ochoa. |
