On June 20, 2024 at 12:01pm, Licensing Program Analyst (LPA) Zirbes met with facility representative Zaray Ochoa. The purpose of the inspection was to conduct a case management deficiencies inspection. LPA disclosed the purpose of the inspection to the facility representative. Present during today’s inspection were 30 preschool age children with three teachers, and three additional administrative/support staff on site.
LPA and facility representative Zaray Ochoa completed a tour of the Center. During a walk through of the outdoor activity space LPA and facility representative Zaray Ochoa observed one pair of adult size scissors on the ground, one empty glass beer bottle and a empty bag of chips. LPA observed preschool age children playing in the outdoor activity space. The scissors and empty glass bottle were 63'9" from outdoor swings where children were playing. A barrier was not present which prevented the children from accessing the above items which are not age appropriate and pose a hazard to the children in care. The items were immediately removed from the outdoor activity space by facility representative. A type B citation was issued for regulation, 101238.2 (d)(2) Outdoor Activity Space, refer to LIC 809D.

A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Facility Representative Zaray Ochoa.

Per facility representative daily outdoor checks are conducted. The items were removed during this inspection. Facility will conduct a staff training. A copy of the staff training will be submitted to the Department
