On November 29, 2023, Licensing Program Analyst (LPA) Zirbes conducted a follow-up complaint inspection to Ready Set Grow Learning Services. LPA met with facility representative Zaray Ochoa. The purpose of the inspection was to deliver the findings for the above complaint allegations. The investigation included an inspection of the facility, a review of facility records, and confidential interviews with staff and parents.
On September 6, 2023, the Department received two allegations. One allegation alleged that on multiple occasions a preschool classroom was observed with one teacher and 14 preschool age children. The second allegation alleged that a staff member was exposed to COVID 19 or had COVID 19, and the Center was not following the COVID 19 guidance.
While investigating the allegation regarding the Center being out of ratio, LPA conducted unannounced inspections. On October 2, 2023, at 2:45pm LPA observed one teacher supervising 15 preschool children. On October 9, 2023, LPA observed an unqualified teacher alone and supervising ten preschool age children.

Based on LPA observation, the Center was out of ratio on October 2, 2023, and October 9, 2023.

Regarding the second allegation involving the Center failing to follow COVID 19 guidelines. Staff interviews confirmed staff 1 (S1) was exposed to COVID 19 on September 6, 2023. LPA completed staff interviews on September 11, 2023. Confidential interviews reported S1 was aware of being exposed to someone with COVID 19 and S1 was showing signs and symptoms of COVID 19. During LPAs staff interview on September 11, 2023, S1 was not wearing a mask. During the investigation, LPA reviewed the California Department of Public Health Guidance on Isolation and Quarantine dated August 29, 2023. Per guidance, persons who are exposed to someone with COVID 19, should wear a well-fitted mask around others for a total of 10 days, especially in indoor setting. LPAs staff interviews were conducted within 10 days of exposure and S1 was not wearing a mask. Furthermore, the Center self-reported a COVID 19 outbreak on September 18, 2023. Based on LPA observation and record review, the Center failed to follow the COVID 19 guidelines.
Based on LPA observations, interviews conducted and a review of the records, the preponderance of evidence standard has been met. Therefore, the above allegations are found to be SUBSTANTIATED. A citation was not issued on the LIC 9099D for California Code of Regulations, Title 22, Division 12, Chapter 1, regulation 101216.3 (a) Teacher-Child Ratio as citations were previously issued on October 2, 2023, and October 19, 2023. California Code of Regulations Title 22, Division 12, Chapter 1, regulation 101223 (a)(2) Personal Rights is cited on the attached LIC 9099D.

Notice of Site Visit was given and must remain posted for 30 days. An exit interview was conducted, and the report was reviewed with facility representative Zaray Ochoa.
