On date and time listed above, Licensing Program Analyst (LPA) Aman Lama conducted a "Case Management- Incident" inspection. Upon arrival, LPA was met with site director, Danielle Alcala and licensee, Kristy Rowell. LPA toured the facility and took census.

It was reported that on 09/23/24 during transition from outside to inside, 2 out of 15 children were left outside on the play yard by themselves with no supervision, including visual observation. Staff noticed the 2 children were missing during the time all the children were divided into their respective classes. The director immediately began looking for the missing children, whom she found outside "laughing and playing on the play structure". Parents were informed and an Unusual Incident Report (UIR) was sent to the Riverside Regional Office (RRO).

LPA conducted interviews with pertinent parties and reviewed documentation. The facility submitted a self-reported incident and also investigated the incident on their behalf. The facility has began necessary changes as well as held a discussion with staff on how to improve, moving forward. Additionally, it was mentioned that there will be a more in-depth meeting with all staff on the importance of supervision and how to improve in this particular area. Staff have also began incorporating more supervision during this transition time, to ensure an incident like this does not occur again.

Due to there being an absence of supervision to the children in care, a Type A citation is being issued.

Facility agrees to hold a meeting, involving knowing the "Responsibilities for Providing Care and Supervision". Director will submit proof of an agenda, with date and detailed description of what will be discussed. Facilty agrees to get signtures from all staff attending and keep on file for furture review.

Based on facility admission, 2 children were left without the supervision, including visual observation of staff. This poses an immediate risk to the health and safety of children in care.

LPA Aman Lama informed the Director, Danielle Alcala to provide a copy of this licensing report dated September 25, 2024 that documents any Type A citation(s) to parents/guardians of all children currently enrolled, or newly enrolled by the next business day or the next day the child(ren) is(are) in care. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification and kept on file for 12 months from the date of this report.


The director, Danielle Alcala was also informed that this report, dated September 25, 2024 which document(s) (1) Type A citation(s), shall be posted for 30 consecutive days as there is(are) immediate risk(s) to the health, safety, or personal rights of children in care.


A copy of this report must remain on file for 3 years.

LPA conducted the exit interview and reviewed this report with site director, Danielle Alcala and licensee, Kristy Rowell .

A notice of site visit was also given and must remain posted for 30 days. Failure to do so can result in a $100 civil penalty.
