Licensing Program Analysts (LPA), Aman Sharma conducted a required annual inspection. LPA was met with assistant director, Melissa Kepner. An inside out tour of the facility was given, and the following was observed and/or noted:
This is a combination center, the other programs are preschool and school-age which were also inspected on this date.
A review of both, staff and children's records were reviewed as part of this evaluation.
The licensee/director is asked to update the following documents, if applicable, and submit to licensing within 30 days:
1. LIC 500 Personnel Report
2. LIC 610 Emergency & Disaster Plan
3. Parent Handbook/ Program Curriculum/Admission policies and procedures/ fee schedule (only if changes have been made)

·The facility is operating with the limits as stated on the license.
· Ratios were met during this inspection
· Classrooms are adequately equipped with age and size appropriate furniture and equipment and are free of hazards.- SEE LIC809D
· There are no weapons present at the facility
· There are no accessible bodies of water present. All wading pools or similar product must be emptied immediately after use and stored in an upright position.
· Children bring their own water bottles to utilize in both, indoor and outdoor activity spaces.
· Uncontaminated drinking water was observed and is used to refill water bottles, as needed
· Medications are being appropriately administered at the facility.
· Hazards are also stored where they were accessible to children. These include: Disinfectants, cleaning solutions and other items that are dangerous to the health and safety of children in care.
· Poisons and toxins are locked.
· All floors were observed clean and safe and shall remain so at all times.
· Restrooms were observed to be sanitary and in operating condition.
· Playgrounds are enclosed by appropriate fences and were observed safe.
· Outdoor activity areas are in good condition and supplied with age/size appropriate equipment.
· The areas around or under high climbing equipment, swings, slides, and similar equipment were cushioned with material that absorbs a fall.
· Food preparation area is clean, free of litter, rubbish and free of rodents and other vermin
· Food is stored appropriately and protected from contamination
· All storage containers for solid waste, including moveable bins have tight-fitting covers that were observed to be on and in good repair.
· Menus were posted with date included and were placed in a visible location of children’s authorized representatives. Menus shall be kept on file for 30 days, and made available upon request.
· Sign in/Sign out record was reviewed and meets regulation requirements.
· Staff record review indicated that staff have current Pediatric CPR/First Aid on file.
· Director completed Health and Safety Training, which is on file.
· A review of children and staff records were found to be complete during this inspection.
· Disaster drills are to be conducted every six months – last drill was conducted on 03/20/23

·The assistant director was informed of their reporting requirements and is provided with the Regional Office’s Unusual Incident Reporting email mailbox: UnusualIncidentReportsDO09@dss.ca.gov
·A review of staff records indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.
· The assistant director/director can submit transfer forms to associate or disassociate someone from their facility at: Associations_Disassociations862@dss.ca.gov
· A review of staff records indicates that staff that were present meet minimum qualifications for the position for which they were hired.

- LPA discussed the safe sleep regulations with assistant director and discussed the Child Care Licensing Safe Sleep webpage as an additional resource at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep

-LPA also informed assistant director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

- This facility does provide Incidental Medical Services (IMS). LPA reviewed storage of medication, equipment/supplies and children’s personnel and administrative records. - The facility is reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

A Notice of Site Visit was issued and must remain posted for the next 30 days, along with a copy of all Type A deficiencies cited during this inspection. A copy of all Type A deficiencies cited during this inspection must also be immediately (within 24 hours of child’s next day in care) given to the parents of all children enrolled in the child care facility and any children enrolled into the child care facility over the next 12 months (at the time of enrollment). Assistant director/director is required to have all parents sign and date the Acknowledgement of Receipt of Licensing Reports (LIC9224) and maintain a copy in each child’s file. A copy of this report, LIC9224 and Appeal Rights (LIC9058) were provided during this inspection.



This report must be available for review, upon request, for the next 3 years.

An exit interview was conducted and this report was reviewed with the assistant director, Melissa Kepner.

Based on observation during the facility tour, LPA found that the licensee did not comply with the section cited above. LPA observed a "Fisher Price walk bounce and ride pony" walker in the infant classroom. This poses an immediate health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in 4 out of 4 persons. The facility was under the impression that 15 minute checks were only required up until the age of 12 months. There were no 15 minute checks found in those files of children between the ages of 12-24 months. This poses/posed a potential health, safety or personal rights risk to persons in care.
