25 | Licensing Program Analysts (LPAs) Susan Brewer and Raymond Moorehead Jr., arrived to the facility to conduct a case management follow-up visit, in response to the receipt of an unusual incident report (UIR) from the facility. An Initial visit was conducted on 03/22/2023 and follow-up visit conducted on 04/11/2023, which were extended due to additional information and interviews needed. UIR was received by the licensing agency on 03/13/2023, regarding possible inappropriate contact between 2 children, and additional children involved. LPAs met with Director Kim Smith and a census was taken of 134 children in care.
On today's visit, LPAs conducted interviews with children and staff, toured the facility inside and out and made observations of the area/(s) where the alleged incident may have taken place. Based on information gathered through interviews, there were conflicting statements provided, regarding inappropriate touch, interaction between children, possible incidental touching and the alleged locations of where an incident could have taken place. LPAs did not observe any obvious blind spots or barriers in the indoor/outdoor open areas that would prevent staff from supervising children. The facility conducted their own internal investigation to address the alleged incident and self-reported the incident to the department, in a timely manner. Therefore, it was determined that no violations have been identified and no citations were issued.
Although, no violations were determined on today's date, LPAs Susan Brewer and Raymond Moorehead Jr., reviewed Title 22 Regulation 101229 Responsibility for Providing Care and Supervision, as a reminder with the facility Director Kim Smith.
An exit interview was conducted and a copy of this report was provided to Kim Smith. |
