On 06/07/2024 Licensing Program Analyst (LPA) Justin Giese made an unannounced visit to the facility to follow-up on the submission of an Unusual Incident Report (UIR). The UIR outlined a alleged incident which occurred on 05/22/2024 pertaining to Personal Rights. LPA met with Administrative Supervisor, Cheryl Soares and discussed the following.

The UIR stated that on 05/22/2024 a pertinent individual had reported to facility management they observed staff in classroom 93 speak in a harsh tone while guiding a child across the classroom. No further details regarding the names or likeness of the individuals involved were disclosed.

On 05/24/2024 LPA made an initial unannounced visit to the facility for the purpose of investigating the submission of the UIR. At time of this visit LPA met with Administrative Supervisor (AS), as well as conducted staff interviews and reviewed pertinent documents. LPA's investigation into this matter did not reveal any corroborating statements or fact to support what was alleged in the submitted UIR.

There due to conflicting information reviewed/recorded during the investigation from what was alleged to have occurred in the UIR, the incident may have happened, or is valid, but there is no preponderance of evidence to prove the alleged incident did or did not occur, therefore the incident is unsubstantiated.

An exit interview was conducted, LPA provided Administrative Supervisor, Cheryl Soares with a copy of this report and a notice of site visit on 06/07/2024.

Notice of Site Visit must remain posted for the next 30 days.
