Licensing Program Analyst (LPA) Taityana Benson arrived at the facility to deliver the findings of the above complaint allegation. LPA was greeted by Teacher Alyssa Trujillo upon arrival to the facility. LPA Benson conducted a tour of the facility inside, outside, and took a census. LPA Benson met Director Sophia Gillyard who arrived at the facility at 8:40 a.m. An in-person 10-day inspection was initiated by LPA Benson on June 11, 2025. During the initial inspection, LPA Benson met with the Director Sophia Gillyard and Assistant Director Marvella. During the investigation, interviews were conducted with pertinent parties and documentation was collected.

On June 06, 2025, a complaint was received alleging lack of supervision resulted in day-care children putting objects in their mouths. During the investigation, interviews were conducted with pertinent parties, documentation was collected, and records were reviewed.

The investigation revealed that there were no reported incidents or injuries resulting in a lack of supervision occurring within the months leading up to the day the department received the complaint. Pertinent parties stated the facility has adequate staffing and supervision of children in care is always maintained. It was disclosed that the facility has not received concerns about maintaining supervision. Other pertinent parties disclosed due to lack of supervision; an incident occurred involving a child. It was discussed that the facility is not adhering to its written policies and procedures regarding supervision. Furthermore, other pertinent parties stated they witnessed a day-care child put objects in their mouth but immediately redirected the day-care child and encouraged the day-care child to use objects for their intended purpose. LPA is unable to determine that there was a lack of supervision resulting in a day-care child putting objects in their mouth.

Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated

A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.

Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

An exit interview was conducted, and the report was reviewed with the Director Sophia Gillyard.
