32 | The diaper rash appeared to be an ongoing problem on and off during the week in question. The rash did worsen after it was reported that child was bleeding, but it is unclear if the rash became more irritated while in care due to staff not changing the diaper as needed or while the legal guardian was transporting the child home with a dirty diaper. It is also unclear, what cause the rash to worsen. During the course of the investigation there was no other information obtained that corroborated the allegation. It was also alleged that the facility is malodorous. During this and previous inspection multiple LPAs verified all rooms at the facility due to facility being under new management which led to construction improvements such as roof repair, paint, and cosmetic work due to the age of the building. This licensing agency is aware that constructions improvement in the ceiling have left a lingering odors due ceiling repair, in addition to diaper changes disposed in the room on daily basis. However, facility has been inspected by companies to rule out any health and safety issues due to odors in the rooms and building was found to be in compliance. Facility director stated that changes have been in place to dispose diapers outside the room to eliminate odors.
Due to the conflicting information and based on the information gathered, there isn’t enough evidence to collaborate the reported allegations. Therefore, based on interviews with pertinent parties and records reviewed, the above allegations are deemed unsubstantiated, at this time. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur.

A Notice of Site Visit was given and must remain posted on, or immediately adjacent to the interior of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
