Licensing Program Analyst (LPA) Becky Mann conducted an unannounced visit to the facility to initiate a complaint investigation. LPA met with J. Rosa Tucker, Program Director and explained the purpose of the visit. The investigation consisted of LPA pertinent record reviews, observations and interviews with staff and clients.
The allegation that Client sustained an injury as a result of an unsafe transfer. LPA interviewed Client #1(C1) they did not recall an unsafe transfer by staff that resulted in an injury. LPA interviewed two (2) staff, they denied the unsafe transfer of client that resulted in an injury, they stated that no such incident has happened.

The allegation that Staff did not seek medical attention for client in care. LPA interviewed C1, they did not recall the unsafe transfer by staff that resulted in an injury. C1 said that they did not need medical attention, they were not hurt. LPA interviewed two (2) staff, they stated that no client was injured due to unsafe transfer, there was no such incident that occurred. If there was, staff would seek medical attention for the clients.

Based on evidence obtained during this investigation, the allegations above are Unsubstantiated; meaning that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur. An exit interview was conducted where this report was discussed and a copy of this report was provided to J. Rosa Tucker, Program Director at the conclusion of the visit.
