| Facility has sufficient non-perishable and perishable food supply for the number of clients in care. LPA observed emergency food supply.
Facility has sufficient care staff for coverage 24 hours a day, 7 days a week.
LPA reviewed client file which contained admission agreements, updated physician reports, individual plan (IPP), and Medication Administration Records (MAR’s). Medications appeared to be administered as prescribed by their physician. LPA also reviewed P&I and client financial log balance.
LPA reviewed staff file for First Aid/CPR certification, training, health screening and clearance which appeared to be current.
Emergency and disaster plans were reviewed. LPA advised administrator it must be reviewed annually even when there are no changes. Fire drill was last conducted on April 25th, 2026, and earthquake drill was last conducted on March 14, 2026. Surety bond is current and expires March 17th, 2029. Liability insurance was reviewed and expires on December 12th, 2026. First aid kit and CPR manual were observed.
Based on the observations made during today’s visit the following was observed and were cited.
· LPA observed expired canned tuna, pumpkin sauce and dry macaroni.
· LPA observed resident roster not updated.
An exit interview was conducted, and this report was discussed and provided to Criselda A. Miguel at the conclusion of the visit with appeal rights.
