On Tuesday, June 23, 2026, at 2:55 pm, Licensing Program Analyst (LPA), Giovanni Cristales met with licensee Tammy Johnson and conducted an unannounced inspection with the purpose to concluded and delivering the findings on the above allegation(s). Upon arrival LPA observed licensee and two (2) children in care. LPA observed the facility to be within ratio. This facility has converted to a small family childcare home and operates 23 hours a day.

Community Care Licensing (CCL) received a complaint on 4/20/26. Throughout the course of the investigation, LPA conducted two inspections at the facility. During the initial inspection on 4/24/26, LPA conducted a joint visit and completed interviews with facility staff (Licensee and co-licensee). Staff interviews did not provide evidence to corroborate with the allegation. LPA collected a copy of the facility roster LIC 9040.

On 6/04/2026, LPA continued the investigation by conducting additional telephone interviews with relevant parties. No disclosures were made alleging that allegations are true.

This agency has investigated the complaint alleging (Staff yell at children, Staff hit children). Although the allegation(s) may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegation(s) are Unsubstantiated.

No deficiencies during today’s visit. A Notice of Site Visit was given and must be posted for 30 days during the hours of operation after each site visit by a licensing representative.

Failure to maintain posting as required will result in a civil penalty of $100.00.
