Bathroom: It was observed to be clean, with soap, toilet paper, functioning faucet sink (cabinets underneath, locked), flushing toilet, Shower/tub and there were no hazards. The following are inaccessible: Sharp items, mouthwash, shampoo, razor, and nail polish.
Kitchen: Sharp items (knives in kitchen cabinet upper cabinet) are inaccessible, and the home has a clean fully stocked food refrigerator/freezer. No chemicals in the kitchen were observed to be accessible. Licensee serves Breakfast, lunch, dinner, and snacks.
Outdoor: The LPA's toured the back yard and observed age-appropriate play equipment, the surface is covered with artificial grass and cement, and the yard is completely fenced in (brick), there are no bodies of water. LPA’s observed plays structure (slide/swing) anchored to the ground. LPA’s observed proper playing equipment.
Advisory/Other: First Aid kit was observed with supplies readily available underneath bathroom sink. The licensee has CPR/First Aid expires: 03/30/2026 and the Mandated Reporter expires: 02/13/27. Electrical outlets are inaccessible. There are no window string/cords accessible to children. Fireplace made inaccessible with glass doors. LPA’s did not observe a sleep log for infant in care. LPA’s reminded licensee that sleeps log are for children 0 – 24 months old. Facility has been cited a Type B Citation. Please see LIC809-D for deficiency page. Licensee will send proof of completion of safe sleep logs no later than 07/10/25.
Per licensee transportation is provided to and from school. LPA’s observed D.L.,Insurance, and registration.
