On 10/02/2025 LIcensing Program Analyst Ana Rodriguez amended facility evaluation report dated 8/1/2025 for the purpose to correct the information provided concerning the requirements for bodies of water.
On 8/1/2025 at 9:30 a.m., Licensing Program Analysts (LPA) Ana Rodriguez conducted an unannounced annual inspection at Wickramarach Family Child Care. Upon arrival, LPA was greeted by the licensee, Gayesha Wickramarach, who provided a facility tour. The home is occupied by 2 adults (the licensee and her husband) and 1 minor child. Seven childcare children were present during the inspection. The licensee stated that the facility operates from 5:00 AM-8:00 PM Monday through Friday. Incidental Medical Services (IMS) were discussed, and the licensee confirmed that no children currently require IMS.
The home is set up as follows:
This is a two-story house with 3 bedrooms, 2 bathrooms, flex room, loft, a living room, a dining area, kitchen, a laundry room, and a garage. Body of water is located on the right of the home in the backyard. The home was inspected for safety, comfort, cleanliness, telephone service, and ventilation. It has central heating and air conditioning, and all windows have screens free of cracks, bugs, and debris.
Main Area: The living room and flex room are for childcare. Children can access bathroom #1.
Living Room: LPA observed age-appropriate toys and furniture in the designated childcare areas. There is child-size furniture and several plastic storage bins containing games and toys for the children. The facility also has games and books available. Mats with educational and learning activities are on the floor and were noted to be in good condition. Two adult-size couches are present, and a TV is used to play educational videos for the children.
Flex room: The flex room is used for napping. LPA observed 1 playpen, 3 cribs, and 4 cots. Children will nap under adult supervision in designated area. The licensee does not provide overnight care.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Children's Bathroom (#1): The children’s bathroom is opon entry to the home to the right. LPA observed that the toilet and faucets are clean and operable. The medication cabinet is free of medications or personal items. The bathroom was found to be clear of any hazardous items.

Off-limit: The front yard, entire upstairs (barricaded by safety gate), backyard, laundry room and garage(key lock door knob) are inaccessible to the daycare children.

Poisons, and medications are not accessible to children. Sharp knives and utensils are stored in the pantry made inaccessible to children with safety latch.Backyard: The backyard was inspected. Licensee stated that the backyard will be temporarily made off-limits and inaccessible to daycare children until the fish pond located in the backyard is fully fence and complies with regulations. LPA obtained signed LIC855-Declaration from Licensee. Fish pond is approximately 9 feet wide by 18 feet long and 1 1/2 feet deep. It is sorounded by rocks and contains 6 fish The backyard is gated all around. LPA observed a grass and concrete area. LPA provided PIN 25-01-CCP.

Additional Information:

Bodies of water: LPA observed an in-ground fish pond in the backyard. It is missing the following requirements: The Mesh/ Chain link/wood/lattice/wrought iron fences constructed at least five (5) feet high. The bottom of the fence is no more than two (2) inches from the ground. The fence does not obscure the fish pond from view. Gates must swing away from the body of water, self-close, and have a self-latching device no more than six (6) inches from the top of the gate. The licensee understands and agrees that the fencing will remain in place whenever licensed care is provided, so long as the fence makes the fish pond inaccessible to children, as determined by licensing staff. No items around the perimeter of the fence would allow the fence to be climbable.
An AC/Heating unit is located on the left side of the home and is inaccessible to children. All unused electrical outlets are plugged and inaccessible to children. The licensee provide breakfast, lunch, and snacks. A required fire extinguisher (2A10BC) was observed to be mounted on the wall near bathroom #1. It is reading in the green zone, inaccessible to children, and meets standards established by the State Fire Marshal. If a child shows signs of illness, they will be separated from other children and stay in flex room. Detergents and cleaning compounds are stored in an off limits garage.
