On August 6, 2025, at 08:04AM. Licensing Program Analyst (LPA) Kendal Zirbes conducted a follow up complaint inspection to Oak Tree Learning Center. The purpose of the inspection was to deliver the findings regarding the above complaint allegation investigated by LPA Joselito Del Mundo. Upon arrival, LPA met with the Director Brea Reid and was granted access to the facility. During this visit, LPA observed 18 children with two staff providing care and supervision. The investigation consisted of interviews with children, staff, parents, director, and review of relevant documents.
During the investigation, facility roster and attendance sheets were provided by the director. Based on confidential interviews with relevant parties, supporting documents, and LPA’s observation, there was not enough evidence to validate the allegation, facility staff do not keep classrooms clean and orderly.
Confidential interviews revealed that the staff use the diaper changing table when changing a diaper for a child which is located inside the bathroom. It was also revealed that the staff use gloves and changing pads during diaper change, and the soiled diapers are properly stored in the trash bins.

Based on LPA’s observation and supporting documents, toys and other learning materials are properly stored inside the cubbies. Also, the facility was observed to be clean and organized.
Although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove the allegation, therefore the above allegation is deemed Unsubstantiated. No deficiencies were cited during this inspection.
An exit interview was conducted, and a copy of this report was provided to Director Brea Reid, along with Notice of Site Visit and Appeal Rights.
