32 | LPA observed new retractable mesh gates installed in each door, safety door latches, and all door alarm sensors were replaced and operable. LPA observed and heard exit doors having a different chime sound from the classroom doors.
On June 12, 2026, a child wandered outside of the facility without staff supervision. Staff noticed that the child was missing from the classroom and started searching. The child’s authorized representatives (parents) were on their way to pick up the child and a sibling from the facility. The child’s parents observed the child wandering alone on the sidewalk with cars passing and went to pick up the child safely removing them from the area. The parents then went to the facility as usual to pick up their child. At that time staff did not know that child was in the parent’s possession until advised. This absence of supervision is a violation; staff are required to visual supervision at all times of children in care.
Evidence from the interviews was obtained to corroborate with the allegation(s) that a daycare child wandered away from the facility due to staff neglect. Although staff members not admitting neglect and instead stating that the child is very skillful, the fact of the matter is that a child indeed wandered away from the facility, therefore corroborating with the allegation. During the time when the child wandered out of the facility the centers door sensors were not working properly, and the far end gate had a door latch which was at the child’s reach and accessible. LPA measured and determined how far the child left from the facility. LPA measured 150 feet from the gate to the end of the facility (street), and 152 feet from the end of the facility to where the child was found. The child wandered approximately 302 feet. Staff interviews revealed that the child went missing approximately 10 minutes.
Based on LPA observations and interviews which were conducted the preponderance of evidence standard has been met, therefore the above allegation(s) that daycare child wandered away from the facility due to staff neglect is found to be SUBSTANTIATED. California Code of Regulations, CCR 102229(a)(1), are being cited on the attached LIC 9099D.
The center will issue one Type A violation, and a $500 civil penalty. Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
Exit interview conducted and a copy of this report was given and explained with director Brea Reid.
