On 10/26/2023, Licensing Program Analyst (LPA) Calloway met with the Site Supervisor, Valerie Villalobos for the purpose of conducting an unannounced Annual/Random inspection. The School Aged Day Care Center program is in the library of Ralph Baker at 21st Century Learning Center. The daily hours of operation are from 2:30 PM- 6:30 PM. Center utilizes the library (Monday- Friday) and the STEM room next door (Monday and Friday only). Teacher/Child ratios are met according to Title 5 of the California Code of Regulations and Title 22 Regulations. Children enter through the front entrance of the center. Upon arrival at 1:45 PM, LPA observed no children in care there was seating for at least fifty (50) children with tables, chairs, and books. Capacity is for sixty (60) day care children. Ages Kindergarten through Grade 6. There is a staff of four (4) teachers on the premises. Incidental Medical Services (IMS) were discussed. At 2:50-3:00 PM, children arrived at the center for census of forty-three (43) day care children.
LPA toured with Site Supervisor and inspected STEM room and there was age-appropriate tables and chairs in good repair. The center has a curriculum learning program and provides activities and snacks. They use roll out carts and bring them to the rooms to provide activities.

The telephone service was verified. Heating, lighting, and ventilation are adequate. There are individual baskets for children's belongings twenty (20) observed. Drinking water is available via water fountain outside the west door supervision is provided or children bring their own water cup that is labeled with their name. There are fire alarms (fire pull observed in lobby area) and carbon monoxide detectors located in the ceiling along with a sprinkler system. Fire extinguishers observed on the wall at the west door.
The facility provides one afternoon snack. Food kept inside a locked door in a cabinet in the cafeteria. LPA observed an adequate amount of food and snacks. Cleaning solutions and other chemicals are kept in back office of the library (off limits).
LPA inspected and observed clean bathrooms (toilets and sinks). The toilets and sinks are functioning properly and are age appropriate. LPA observed soap, toilet paper, and paper towels readily available. Locked storage cabinet observed. The staff restroom is near the school office in the staff lounge.
LPA observed a stocked first aid kit (fanny pack) fully charged fire extinguishers and carbon monoxide detectors throughout the center. There is a Fire Drill/Earthquake log posted, and a current Roster available.
Children are inspected for illnesses as they arrive. A review of medication policy indicated that prescription medication is administered only with the parent's written permission. The Director administers medication and documents the dosage, date, and time on a log. Medication is brought and taken home by the parent daily. Medication is properly labeled and stored in its original container. There are currently no children in care with IMS needs on this site. There is a separate area for isolation and care of ill children in the library (mat side of the room) away from the other children.
Sign-in and out sheets were reviewed (outside table in book). The Teacher to child ratio was observed and staff names were recorded. Care and supervision were evaluated to determine if the basic needs of children are met and appropriate. LPA observed a parent board consisting of licensing-required postings. Fire/earthquake drills current.
Children's records and staff records were reviewed. LPA verified there is at least one (1) staff person present with current CPR and First Aid Training (exp.10/21/2025). Mandated Reporter was expired or missing in all staff files.
The outdoor play equipment was inspected for health, safety, cushioning material, good repair, and age appropriateness. The play area has concrete. There is an area for rest (canopy). Water is provided via the water fountain. The play area was inspected and found to be free of hazards and inaccessibility to bodies of water. Separate playground schedule for preschool and school aged children. The jungle gym is anchored. There is a basketball court for active play.

The following general information was discussed during this inspection:
The Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Mandated Reporter Training Requirements: §1596.8662 - As of January 1, 2018, childcare providers, administrators, or employees who work in a licensed facility shall complete the mandated reporter training provided according to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. Staff was informed that they need to take mandated reporter training.
Immunization Requirements: §1596.7995 (a)(1) Commencing September 1, 2016, a person shall not be employed or volunteer at a daycare center if he or she has not been immunized against influenza, pertussis, mumps, rubella, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year.
All Licensing reports are available for review online and are considered public information. Summary: Assembly Bill 2621 added Section 1596.819 to the Health and Safety Code, to require the Department to post certain licensing information for CCCs and FCCHs on its public internet website.
Fingerprint clearances and transfers: Before working or volunteering in a licensed childcare facility, all individuals subject to a criminal record review shall obtain a clearance or criminal record exemption. If a fingerprint clearance has been obtained through the Department, Licensee may request a transfer of a criminal record clearance from one state-licensed facility to another using form LIC 9184
Each report (documenting a Type A citation) shall remain posted for 30 days along with the Notice of Site Visit (printed out during this inspection). In addition, all parents of currently enrolled children and any newly enrolled child for the following 12 months shall receive a copy of the report documenting a Type A Citation and sign form LIC 9224 acknowledging receipt placed in child’s files. Staff is aware of the required forms for children's files and forms that shall be posted after licensure.

Senate Bill AB 633 - Child Care Facilities: Parent Notification Requirements
Summary: This bill amends Health and Safety Code (HSC) sections 1596.859, 1596.8595, 1596.8895, and 1597.05 to improve the transparency of licensing records and to ensure that parents/guardians using a licensed childcare facility are aware of situations that present the greatest danger to children. Parent Board contained all documents that are required to be posted according to Title 22 Regulations.

Per Title 22 Regulations, there are deficiencies being cited during this inspection. See 809-D pages.

Exit interview conducted with a copy of this report was read, a Notice of Site Visit was provided to Valerie Villalobos, Site Director at the facility. A Notice of Site Visit must remain posted for thirty (30) consecutive days. Failure to maintain posting will result in a $100 Civil Penalty
