On February 17, 2023, Licensing Program Analyst (LPA) Zirbes met with the Administrator Delores Toner for the purpose of conducting an unannounced required one year inspection. LPA toured and inspected the facility in accordance with the facility sketch. There were 26 school age children and three teachers, one aide and two additional staff members present. Based on LPAs observation, the staff/child ratio was being met and the program is not exceeding the approved capacity for the facility. The school age program is located on on the grounds of Baldwin Lane Elementary School and is operated out of two modulars (B-10, B-11). The program operates from 7:00am-8:30am and 2:30pm-6:00pm Monday thru Friday, Thursday the program opens an 1:00pm .

Indoor/Children's Area: School age center is equipped with appropriate furniture and equipment. LPA observed a art supplies, television, books, games along with multiple tables and benches for school work and other activities. Furniture is maintained in good repair and safe condition (age appropriate). LPA observed the air hockey table in the play room was missing a cover on one of the corners therefore the tops of two screws were exposed. LPA advised the Director to have the corner covered and remind staff to ensure all equipment maintained. Drinking water is available inside the classroom via water cooler and cups. Each school age child has an individual cubbies labeled with their name for the storage of personal items.
During the inspection, LPA observed cleaning products and disinfectants were located in lower cabinet in the kitchen area within classroom B10. The lock on the cabinet was not locked at the time of this inspection, therefore the cleaning products were accessible to the children in care. LPA observed zero children in the kitchen area at the time of this inspection, therefore this was a potential risk to the children in care. A type B citation was issued. The cabinet was immediately locked and the cleaning products were made inaccessible. The Center does not have a fireplace or wood burning stove. The buildings have central heating and air conditioning. The room temperature was comfortable at the time of this inspection. LPA observed the carbon monoxide, fire extinguisher and smoke detector were located in the licensed space. There is a working telephone on the premises. Report continued on page two

If a child became ill during operating hours, the Isolation are was located in modular B-10 in the calm corner Each classroom had a first aid kit. LPA observed sign in/out sheets at the entrance to the facility. LPA observed the staff were signing the children in and out following transportation to school. No smoking is allowed on the premises. There program utilizes the elementary schools bathrooms. The Boys bathroom had three toilets, two urinals and two sinks. The girls bathroom had five toilets and two sinks. The bathroom utilized by the school age children was in safe and sanitary operating condition at the time of this inspection.

School age children are inspected for illnesses (wellness policy) as they arrive. Per Administrator at this time zero medications are being provided.

Outdoor
LPA inspected the outdoor play space for health, safety, cushioning material, good repair and age appropriateness. The School age program utilizes the outdoor play equipment on the elementary campus. The outdoor space was fully enclosed by a chain link fence. LPA observed a variety of outdoor materials such as hula hoops and balls. LPA observed climbing equipment on the playground. The climbing equipment was secured to the ground and was placed on wood chips. Water is accessible to the children via drinking fountain.

Health Related Services
Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. Child/Staff/Personnel Records
LPA reviewed 10 child records and four staff files. Ten of ten child files were missing the LIC 995 and the LIC 613A. One of four staff files had an expired mandated reporter certificate. Three of four staff files reviewed were missing documentation of the mandated reporter training, immunization's, and education requirements.

Facility Records


LPA and facility representative reviewed the LIC 311A Records To Be Maintained At The Facility. LPA observed a current roster, fire/disaster drill log (February 2023). A staff member was on site who had a current CPR/First Aid.
Posting Requirements
The following were observed posted as required: facility license, Personal Rights (LIC613A), Parent’s Rights Poster (PUB 393), car seat law (PUB 269), emergency disaster plan, earthquake preparedness checklist.

Other: There are no recalled and or prohibited toys or play equipment observed on the premises.

Facility representative was advised of the requirement to report Unusual Incidents. Facility representative informed to utilize the Unusual Incident Report/Injury Report LIC624 when submitting the report to the department (unusualincidentreport@dss.ca.gov). A report shall be made to the department by telephone or fax during the department's normal business hours before the close of the next working day following the occurrence during the operation of center. In addition, a written report shall be submitted to the department within seven days following the occurrence of any events specified above. The On-Duty Worker is available for questions at (661) 202-3318 Monday through Friday 8am-5pm.


Any duly authorized officer, employee, or agent of the Department shall, upon presentation of proper identification, shall inspect the facility. The Center shall permit the Department to inspect the child care center and to privately interview children or staff, to determine compliance with or to prevent violations of child care laws or regulations, also enter and inspect any place providing personal care, supervision and services at any time, with or without advance notice, to secure compliance with, or to prevent a violation.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform. LPA discussed the requirements for Lead testing in Child Care Centers.

As a result of this inspection, three type B citation and two technical advisory notices were issued. Refer to LIC 809D. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Administrator Delores Toner.
