Infant File and Staff File Review: LPA reviewed a total of five infant files and four staff files. LPA reviewed two files for children under 12 months of age. On 9.16.24, the safe sleep log documented Child 1 (C1) fell asleep on their stomach and remained on their stomach from 11:45am-12:15am. On 9/17/24, C1s safe sleep log indicated C1 was fell asleep on their side from 9:10am to 9:40am and then 11:00am-11:30am. Per LPAs review of C1 individual infant safe sleep plan dated 8/14/24, section C has not been completed and signed by C1 authorized representative. LPA discussed the record with the staff 1 (S1) and Administrator. Administrator reported they have never observed C1 sleep on their side or stomach. S1 stated C1 is typically a side or back sleeper. During the inspection the Administrator shared that they talked to the staff who's initials were on the log 9.17.24, and they stated they may have made a mistake on the form. At 12:20 pm and 1:30pm, LPA completed a walk through of the infant safe sleep area and observed C1 and other infants sleeping on their back. LPA advised the staff that all children under 12-months of age must be placed on their backs for sleeping and if section C of the infant’s Individual Infant Sleeping Plan [LIC 9227 (3/20)] is not completed, then staff shall return the infant to their back. LPA provide technical assistance regarding safe sleep. Per Administrator and S1 a staff training will take place regarding the 15 minute safe sleep checks and infant safe sleep requirements.
LPA reviewed two child files for children under 12 months of age. Per LPA record review, child 2 (C2) authorized representatives signatures was not obtained on Section D as required. LPA provide technical assistance and requested that all individual infant sleeping plans be reviewed to ensure Section D is completely filled out.
Three technical advisory notices were issued regarding the infant file review. Per LPA review of the staff records, all required information was included.
Facility Records
LPA and Facility representative reviewed the LIC 311A Records To Be Maintained At The Facility.
Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.
For child care center licenses issued after July 1, 2022, the licensee shall test their water for
lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-
CCP).
LPA verified that the lead testing was completed in accordance to the Written Directives
outlined in PIN 21-21.1-CCP. Report continued on page four |
