On September 11, 2025, Licensing Program Analyst (LPA) Giovanni Cristales met with Director Cynthia Vitto who granted access for the Annual/Random inspection for the Infant Component. The facility also has the pre-school (367750064) and School-age (367750065) components on the premises. The childcare center is located in modulars on the grounds of the Generation Church. Upon arrival, LPA observed 7 Infant children and 3 staff. A tour of the facility was conducted inside and outside. The tour included one Infant classroom, kitchen, office space, and the outside play area. The Days/hours of operation are Monday through Friday from 6:30 am to 6:00 pm. Incidental Medical Services (IMS) were discussed. All employees have a criminal record clearance and are associated with the facility.

Physical Plant, Indoor/Children’s Area: Childcare center is clean, safe, sanitary, and in good condition; stairways, ramps, all indoor passageways, and other areas of potential hazards are kept free of obstruction; floors of the classroom has a surface that is safe and clean, cleaning and poisons compounds kept inside children’s bathroom with key lock, furniture/equipment is good condition; tables/chairs provided to meet children’s needs; all play equipment and materials used by children are age-appropriate. Each child has individual permanent storage space (individually labeled with name) for his/her clothing and personal belongings and proper napping equipment also labeled with child’s name.

Trash cans for solid waste have tight-fitting lids. Water is provided by the facility. All materials and surfaces are toxic-free and inaccessible and there are no fireplace or heater units. The classroom has a central A.C. unit with appropriate temperature, working carbon monoxide detectors, smoke detectors, and Fire Extinguishers (3A40BC) last served on 2/3/2025. There are both landline and cell phone services on the premises.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Outdoor: The infant children use a separate small playing area within the large playing area which is fenced all around. There is a large umbrella type canopy area which provides shade and rest. Children’s use of the outdoor play equipment was inspected for health, safety, good repair, and age appropriateness. The entire playing area has rubber flooring to support or cushion a fall. The area was observed to be free of debris, free from hazards, holes, and broken items.

Staff/Personnel Records: Designation of Responsibility observed, immunization's, TB clearance, mandated reporter training, Director qualifications, health screening, criminal record statement, and a statement acknowledging suspected child abuse are available for review.

Facility Records: Roster, Fire/Disaster and Earthquake drill log last completed on 08/14/2025, CPR/First Aid (9/21/2026) and Mandated Report training (09/12/26) were reviewed and maintained current. Sign-in and out sheets were reviewed.

Posting Requirements: Failure to comply with posting requirements shall result in an immediate civil penalty. The following were observed posted as required: Facility license, Personal Rights (LIC613A), Child passenger restraint system poster (PUB 269), Notification of Parent’s Rights (PUB 393), Emergency Disaster Plan (LIC 610), and Earthquake Preparedness Checklist (LIC 9148).

Food Service: The facility provides breakfast, snacks, and lunch. Menus are posted in front of the day-care entrance door. LPA observed an appropriate amount of food and snacks inside the kitchen. Cleaning solutions and other chemicals are kept in the cabinet underneath the kitchen sink and the kitchen is locked and inaccessible to day care children.

Advisory/Other: First aid supplies (thermometer, bandages, scissors) are readily available in the office. There is an isolation area for children who become ill while in care located in the front office. The director also stated that if the child is sick, children are isolated and taken to the staff bathroom and supervised until parents pick up the child. Smoking is prohibited on the premises, and daily inspection for illness is conducted. Firearms/weapons are not allowed or stored on the premises. There are no pools or bodies of water on the premises. Daily Activity Schedule was observed. LPA observed water lead testing (6/8/2022).

Electrical outlets are inaccessible, there are no recalled or prohibited toys or sleep/play equipment that were observed on the premises. There are no window cords accessible to children.

Teacher-child ratios were observed, and staff names were recorded. Care and supervision were evaluated to determine if the basic needs of children were met and appropriate.

Health-Related Services: The Director has been advised that all prescription and non-prescription medications must have the child’s name and are dated, written consent and instruction from the child’s representative, and a plan to document and report to the child’s representative when medication is administered to a child; Medication will be properly labeled and stored in its original container. The IMS plan was discussed, and the Director understands when IMS is necessary

The facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The facility representative was informed of the MyChildCarePlan.org website, a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

All staff are required to take 3-hour Child Care Provider Mandated Reporter training (AB1207) every 2 years. Beginning on January 1, 2018, Assembly Bill 1207 (2015) requires all licensed providers, applicants, directors, and employees to complete training as specified in their mandated reporter duties and to renew their training every two years. Applicants must meet requirements as a precondition to licensure. New employees shall have 90 days from the date of employment to complete training as required. The training may be conducted at the following website www.mandatedreporterca.com. The director is aware self and all staff are mandated child abuse reporters and have the responsibility of reporting any suspected child abuse to the Child Abuse Hotline at (800) 540-4000.

The Licensee advised of the requirement to report Unusual Incidents. The licensee was informed to utilize the Unusual Incident Report/Injury Report LIC624B when submitting the report to the department (email address on the website: www.unusualincidentreport@dss.ca.gov). A report shall be made to the department by telephone or fax during the department's normal business hours before the close of the next working day following the occurrence during the operation of the daycare center. In addition, a written report shall be submitted to the department within seven days following the occurrence of any events specified above. An On Duty Worker is available for questions at (661) 202-3318 Monday through Friday 8 am-5 pm.

Any duly authorized officer, employee, or agent of the Department shall, upon presentation of proper identification, shall inspect the facility. The director shall permit the Department to inspect the family childcare home and to privately interview children or staff, to determine compliance with or to prevent violations of childcare center or regulations, also enter and inspect any place providing personal care, supervision, and services at any time, with or without advance notice, to secure compliance with, or to prevent a violation.

A survey will be sent to the email address provided to improve the quality and value of the new inspection process. Please complete the survey and share your inspection experience. An exit interview was conducted, and the report was reviewed with the Director Cynthia Vitto.
