Licensing Program Analyst (LPA) Maddox met with Director Amanda Haskins and Owner, Cynthia Vitto today to conduct a Case Management - Incident Follow Up Inspection for an Unusual Incident that occurred on 06/30/23. The Unusual Incident was self reported within the time frame specified by regulations. Upon arrival, LPA observed 13 preschool children in care with 2 staff present. LPA Maddox met with Director and Owner regarding the incident, child #1 wasn't present during this inspection.

Description of incident: On 06/30/23, Child #1 was running on the play yard when his shoe came off. As child #1 stopped to put his shoe back on, he lost his balance, fell and hit his head on a picnic table sustaining a cut on his forehead. A staff person immediately brought child #1 to the office, Owner applied pressure to the wound, another staff called 911, and another staff contacted the parents. Mom arrived at the center within minutes (lives close by) and accompanied child #1 in the ambulance to Loma Linda. Child #1 received 3 stiches on the right side of his forehead.

LPA viewed the picnic table with attached benches (hard plastic) and determined it's age appropriate for PS children. LPA did not observed any sharp or pointed edges, and the picnic table is in good repair. LPA will follow up with interviewing child at a later date, no citations were issued at this time.
