Licensing Program Analyst (LPA) Maddox conducted an announced visit today and met with Oscar Mendez, Recreation Superintendent, and Luz Tuchan, Program Director for the purpose of conducting a Pre-Licensing inspection for a School Age (SA) component. Center requesting licensure for 30 children ages T-K through 5th grade. This center is located on the premises of North Shore Elementary School and will occupy 2 modular (Room 29 & 30). The hours of operation will be: Mon through Friday from 7:00 am to 8:30 am (morning session) and from 3:00 pm to 6:00 pm in the afternoon. On Thursdays the school has minimum days so the afternoon session will begin at 1:00 pm to 6:00 pm.

INDOOR ACTIVITY SPACE:

· The child care center was toured and found to be clean, safe, sanitary, and in good repair to ensure the safety and well-being of children, employees and visitors


· Floors of all rooms have a surface that is safe and clean (wood flooring)
· A comfortable temperature for children shall always be maintained.
· Furniture and equipment are maintained in good condition, free of sharp, lose or pointed parts. There are a variety of age-appropriate equipment, toys and materials in good condition and in sufficient quantity to allow children present to fully participate in planned activities.
· Tables and chairs were present to meet the needs of the children.
· Drinking water is readily available inside the classroom water will be delivered, water cooler with disposable cups.
· Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children will be stored in a locked cabinet.

There will be fully stocked first-aid kit(s) in the classrooms with a portable back pack for the play yard.
· The isolation is located in the classroom (in a corner of the classroom) for any child who becomes ill during the day.
· LPA observed operable carbon monoxide detectors and smoke detectors(hard wired). Fire extinguishers are located in each classroom.
· The center has a working telephone
· Sign-in and out procedure manually.
· The licensee shall conduct a wellness check to ensure that children with obvious symptoms of illness including, but not limited to, fever or vomiting, are not accepted.

OUTDOOR

LPA observed a shaded rest area for the children. Easy up pop up tents will be provided for outdoor shade. The play yard was free of hazards including, but not limited to, holes, broken glass and other debris, and dry grasses that pose a fire hazard. There was no climbing apparatus present during this inspection. The yard is 80% fenced, there are 2 openings, LPA is requesting staff place cones in front of the two openings to prevent children from entering areas were staff will not have visual observation. There are no bodies of water on the premises.

RESTROOMS

There are 2 separate bathrooms available for children. Each bathroom has staff to provide individual privacy. All toilets and hand washing facilities shall be maintained in safe and sanitary operating condition. Girls will need escorting to the bathrooms which are located around the outside of the building where staff will not have visual observation. There are additional restrooms available inside the main building if needed (Staff must escort children to the additional restrooms).

SUPERVISION:

Applicant shall ensure no child is left without the supervision of a teacher at any time, Supervision shall include visual observation.

TRANSPORTATION:

Center will not provide transportation.

CHILDREN’S RECORDS:


LPA explained all children's records shall be available to the Department to inspect, audit, and copy upon demand during normal business hours.
NAPPING

Children will not nap at this center, but children will be allowed quiet rest time if needed.


HEALTH RELATED SERVICES:

Staff will not dispense medications.

FOOD SERVICES

The snack menu must be posted for parents’ review. Children will be escorted to the elementary school cafeteria for breakfast, staff will serve afternoon snacks.


· Trash cans, including moveable bins, shall have a tight fitting cover that is kept on; shall be in good repair; and shall be leakproof and rodent-proof.
· There is a refrigerator available to store any medication that requires refrigeration.

POSTINGS:

Applicant was informed of the required forms that shall be posted after licensure including the telephone number of the local health department and information on child passenger restraint systems pursuant to Health and Safety Code section 1596.95(g) and Vehicle Code sections 27360 and 27360.5

Each licensee shall have a disaster and mass casualty plan of action. The plan shall be in writing and shall be readily available.

Applicant is reminded of the requirement to report and unusual incidents and/or injuries to the parent/guardian and Licensing within the time frame specified by the regulation and on the form LIC 624B.
