On 3/19/2025 at 2:10 p.m. Licensing Program Analyst (LPA), Victoria Hernandez conducted an
unannounced case management investigation inspection following an injury incident that was reported on 3/6/2025 Upon arrival, LPA met with Director, Dana Trowbridge and discussed the purpose of the inspection, and was led on a tour of the facility. During the inspection, there were 50 children and nine (9) staff present.

Interviews were conducted with director, and staff members. Copies of documents relevant to the injury incident obtained.

There were no deficiencies were cited during today's inspection.

Exit interview conducted and report was reviewed with Director, Dana Trowbridge. A Notice of Site Visit was given and must remain posted for 30 days.
