Licensing Program Analyst (LPA) Liliana Silveira conducted an unannounced Case Management - Incident visit. LPA was welcomed by, identified herself to, and discussed the purpose of the visit with Director of Programs Thomas Carr and Assistant Director of Programming Mark Louis Fisher.

Today's visit was in response to an LIC624 Incident Report which licensee self-submitted to the CCLD San Diego Regional Office (received on 03/11/2024), involving Client #1(C1) and Client #2 (C2). [See LIC 811 Confidential Names List for a description of person identifiers used in this report].

LPA gathered copies of pertinent records, interviewed staff and briefly spoke to the Directors. No deficiencies were observed or cited during today's visit.

An exit interview was conducted with Thomas Carr and Mark Louis Fisher, to whom a copy of this report, LIC 811 and the Licensee/Appeal Rights (LIC9058 03/22) were provided.
