13 | Licensing Program Analyst (LPA) Ramon Serrano conducted an unannounced complaint visit to deliver findings on the above allegations. LPA met with House Manager Pat Ruis and we discussed the purpose of the visit and elements of the complaint.
On June 1, 2021, the day the investigation was opened, LPA Serrano and LPM John Rante conducted a full inspection of the facility. While inspecting Room #4, LPA and LPM observed a bed bug on the mattress in room #4. Client occupying Room #4 expressed not seeing bed bugs or insects and was unaware there was a bed bug in the room. No other insects or rodents were observed during the onsite inspection by LPA and LPM. Throughout the course of this investigation, LPA reviewed facility records and conducted pertinent interviews. Interviews have revealed that both clients and staff have not seen bed bugs, roaches, or rodents in the facility. An interview with an outside source revealed that a client had observed roaches in the facility at night. Based on the information provided, LPA was unable to identify the eyewitness of the roaches. A review of Terminix invoices revealed the room #4 was treated on May 28, 2021 and no further treatments were needed.
