13 | Licensing Program Analyst (LPA) Renita Hall, conducted an unannounced visit regarding the above-mentioned allegation to open and deliver findings. LPA was allowed entry by Roger Hernandez, Administrator. LPA identified herself and disclosed the purpose of the visit and elements of the complaint to the Administrator.
The Department's Investigation consisted of interviews with staff and residents. LPA conducted a tour of the showers and noticed that three out of four showers were operatable in the single-story building. The facility maintenance staff was called to repair the shower that was inoperable. There was also access to other showers for use.
The Department investigated the allegation as mentioned above and based on observations, and interview, the preponderance of the evidence has NOT been met, due to three of the four showers located in the single-story building were operatable, and there was access to other showers located on the premises for use.
