25 | Licensing Program Analyst (LPA) Jose De La Cruz made an unannounced visit to conduct a Required Annual Inspection. The facility file was reviewed prior to the visit. LPA was welcomed by, identified himself to, and discussed the purpose of the visit with Facility Director Nora Liwag (FD). The facility's license shows a maximum capacity of six (6) elderly residents age 60 and above, all of whom might be non-ambulatory.
LPA arrived at 8:15 am to the facility, and requested staff and resident's records.At 10:50 am, LPA, FD toured the facility, the interior and exterior, six rooms and three bathrooms. The residents rooms where clean and with personal items and memorabilia. Activities and puzzles were found on the living room and residents rooms. The facility was clean, sanitary, and in good repair. Pathways were free of obstruction and slip hazards.The facility had sufficient space and equipment to facilitate dining, laundry, visitation, meetings, and client activities.
The facility contained at least 2 days of perishable food, and at least 7 days of non-perishable food, all safely stored. Cooking, dining equipment, and utensils were present. Toxic chemicals, poisons and sharps were locked in a cabinet.
LPA measured the water temperature in the clients bathrooms and the kitchen sink in Fahrenheit degrees: B1 115.3, B2 116, B3 119, kitchen 118.
