The Department conducted a records review. A medical report for R1, dated 05/19/2018, revealed that R1 is diagnosed with schizophrenia. The LPA interviewed R1 and it was revealed that R1 is not a credible witness. R1 was not able to form coherent sentences and some responses were illogical, such as R1 calling the facility a morgue. Interviews with two (2) care staff revealed that since December 2025, R1 has been changing. R1 is physically weaker, making false claims and acting more paranoid.
The interviews with the two (2) staff also revealed that they have not observed other staff mistreating R1. Interviews with three residents (3) revealed that they have not experienced mistreatment from staff.
There is insufficient evidence to support these allegations. No citations were issued during today’s visit.
The preponderance of evidence standard has not been met, therefore, the above-mentioned allegations are unsubstantiated. An exit interview was conducted with Eveline and a copy of this report, along with Licensee/Appeal Rights (LIC 9058 03/22) were provided to Eveline via email.
