The Department also interviewed an Outside Source (OS1) that advocates for R1. The interview revealed that R1 has consistently refused to take their medications and OS1 is continuously reaching out and encouraging them to take their medications. Finally, interviews with two staff members and the Administrators revealed that they continuously encourage R1 to take their medications and try different strategies to encourage R1 to take their medications. There was not enough evidence to corroborate this allegation.
It was also alleged that R1 sustained weight loss due to staff neglect. The Department interview with R1’s PCP revealed that, regarding the weight of R1, R1 fluctuates with weight due to their COPD issues and cigarette smoking. The PCP stated that R1’s weight is within a normal range and they were last weighed on June 30, 2026. The PCP also stated that the facility goes “above and beyond” to reach out to the resident’s doctors when there are resident issues that they are concerned about. Finally, interviews with OS1, staff and the facility Administrator also revealed that they did not observe any substantial weight loss issues with R1 that were beyond normal. There was not enough evidence to corroborate this allegation.
The preponderance of evidence standard has not been met, therefore, the above-mentioned allegations are unsubstantiated. No citations were issued during today’s visit. An exit interview was conducted with Eveline, and a copy of this report, along with Licensee/Appeal Rights (LIC 9058 03/22) were provided to the Eveline via email.
