32 | Licensee Representative, Facility Representative and Staff #4 (S4) stated that the facility has quarterly pest control inspections and mouse traps and glue traps were used to prevent the recent problem.
On 1/13/2025, 1/28/2025 and 2/25/2025, during inspections of the facility, LPA did not observe any infestation in the kitchen, classrooms or any of the daycare areas, the facility appeared to be clean and orderly.
Based on interviews conducted with licensee representative, facility representative, staff members, daycare children, daycare parents and LPA’s review of pest control service invoices, there is not enough corroboration and evidence regarding the allegation. Due to conflicting information obtained from the interviews, although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the above allegation is found to be unsubstantiated.
A Notice of Site Visit (LIC 9213) was given to Facility Representative, Sally Gutierrez. and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Appeal Rights (LIC 9058) was provided. Exit interview conducted and report was reviewed with Facility Representative, Sally Gutierrez. |
