On 9/25/2024 @ 1:05PM, Licensing Program Analyst (LPA) Nancy Diaz conducted an unannounced inspection. The purpose of this inspection is to follow-up on 2 self-reported incidents.

LPA met and toured the facility with Patty Rea, Associate Executive Director & Lee Ann Chavez, Supervising Teacher. Observed present today were 114 preschool children in 6 classrooms. Appropriate ratios were observed in all the classrooms.

The incident that occurred on 9/18/24 involved a child who placed a metal nut inside the ear. The metal nut came out while child was on the way to see a doctor. Staff removed the small pieces of nuts and bolts from the classroom. Children were also reminded not to place any foreign objects in their ears.

The incident that occurred on 9/17/24 involving a staff and two children is currently under internal investigation. LPA interviewed a total of 2 children. LPA was unsuccessful interviewing the 2nd child (age 3) due to child's inability to communicate and was also observed to be easily distracted.

Due to insufficient information available at this time, the second incident needs further follow-up.

There were no deficiency observed today.

Exit interview was conducted with Ms. Rea. A copy of this report, appeal rights and Notice of Site visit were provided. The notice of site visit must be posted for 30 days.
