13 | On February 27, 2024, at 10:06 AM Licensing Program Analysts (LPA’s), William Chancellor and Courtnee Peebles arrived unannounced to TRI-CITY PRE-SCHOOL (CCC) and met with Director (DIR), Laura Katie Donahue to initiate a complaint investigation into allegations listed above. LPAs conducted a tour and census of the CCC. During the investigation, LPAs conducted confidential interviews with five staff (D), (S1), (S2), (S3), (S4).
On February 21, 2024, a complaint was received with an allegation stating, CCC Staff left child in care unsupervised, and staff did not follow reporting requirements. Five of six interviews disclosed, that C1 had been left alone, inside of the classroom for approximately 30 minutes, while the rest of the classroom went to music class in the gymnasium. C1 was not found until teachers and class returned from music class. Additionally, administration staff received a memo and individual training on how to ensure no child is left unattended during transitions the next day. Staff immediately removed blind spots from classroom library and improved policy to do a full walk through before leaving an area. Furthermore, CCLD did not receive an unusual incident report for this specific incident, LPAs informed DIR about the importance of reporting requirements and notifying CCL.
