7 | 101226 Health-Related Services (a) The licensee shall immediately notify the child's authorized representative if the child becomes ill or sustains an injury more serious than a minor cut or scratch. The licensee shall obtain specific instructions from the authorized representative regarding action to be taken. (2) In the case of less serious injuries including, but not limited to, minor cuts, scratches and bites from other children requiring assessment and/or administration of first aid by staff, the licensee shall document the injury in the child's record and notify the child's authorized representative of the nature of the injury when the child is picked up from the center. 7 | Director shall include a training by 2/23/24 that includes directions to staff members on how to fill out and deliver incident reports for parents. Incident reports shall be delivered and communicated to parents same day of the incident at pickup. |

14 | Based upon interviews with Director, staff, complainant and parents, parent of C2 was not notified of the second bite upon pickup from the center. Other parents also stated that notification of injuries and behavior issues (biting) are not consistently communicated or received from the facility which is a potential health, safety and personal rights risk to children in care. | 8

14 | Incident reports should be consistently written for both the biter and child that was bitten and records kept to know which child was bit and when. Director will provide a signed staff agenda dated by 2/23/24 and email to LPA Lane by EOB on 2/23/24. |
