7 | (d) Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event. 7 | Assistant Director will provide an unusual incident report for other incident that occurred in March where child was possibly left unattended. She will ensure that in the future incidents are reported on the duty line (619)767-2248 within 24 hours and will submit an incident report by email to SDIncidentReports@dss.ca.gov or by Fax at (619 767-2203 within 7 days of occurring. Assistant Director will also provide written statement explaining that she will submit future reports per the required time frames. |
