25 | On 2/17/23 at 9:40 AM, Licensing Program Analyst (LPA) Keturah Lane conducted an unannounced proof of corrections (POC) visit to the facility. Upon arrival, LPA was greeted by Director Celida Banuelos and toured the facility. Census was total 33 children as follows:
- Preschool room had 17 children with staff members: Iris Meza, Donna Karmakar and Sara Elzahabi
- 2-3 yr old classroom had 16 children with staff members: Noemi De Dominguez and Jahavi Gurkhe
Prior to the visit on 2/13/23, LPA received a corrections policy via e-mail from the Director that stated the sensory table should be inspected prior to use and no rubber mulch should be used for pouring or in sensory table. The document also stated that rubber mulch/wood chips are ONLY for fall zone and should not be used for activities, pouring or play. Staff members were made aware of the updated policy and signed the form.
During the visit, LPA observed the playground for the 2-year olds that has the rubber mulch for the playground surface. LPA observed new equipment and materials for the children to play with. LPA also observed that all cups, bowls and pouring devices were removed from the playground. The sand/water table was also removed from the playground and did not have any rubber mulch in it. The sand/water table was located in an outside walkway on the way to the playground and contained cornmeal and dinosaurs inside. LPA interviewed Staff member #1 on the playground. Staff #1 stated that they received 4 new bikes and 6 balls for the children to play with and that the new playground rules are a plus because the children are more active and not sitting and playing with the rubber mulch as much. Staff #1 stated that the Director acted quickly to correct the citation. Staff members were aware of the new policy. Director stated she has purchased additional equipment and materials for the playground and that a proposal has been submitted to "revamp the 2's playground" and possibly replace the rubber mulch with another suitable cushioning material.
Type B deficiency has been corrected and cleared. Notice of clearance was provided to Director. Exit interview conducted and report was reviewed with facility representative Director Celida Banuelos. A notice of site visit was given and must remain posted for 30 days. |
