On July 8, 2022 ,at 11:21 AM, Licensing Program Analyst (LPA) Cindy Hamilton arrived at Grace Lutheran Preschool (CCC) and met with Director Darlene Beebe and Superintendent Ben Elliott, regarding information received during an inspection on 04/07/2022.

On 04/07/2022, LPA Hamilton conducted interviews and LPA was informed that there had been two incidents in the Terrific Toddler classroom where children were not properly supervised. One incident involved, Child #5 (C5) leaving the classroom on or around April 6, 2022. C5 was observed by Staff #3 (S3) in the hallway while S3 was taking S3’s class to the restroom. S3 advised Staff #5 (S5) who was in the classroom alone with approximately 13 other children, while the other teacher was using the restroom, that C5 was in the hallway. Another incident involved an unknown child being on the playground with no supervision and it was brought to the CCC’s attention by a parent that was at the CCC.

Based on information gathered, C5 was not injured and did not venture far from supervision as there was actually two teachers out in the hallway at the time to redirect C5. In addition, C5 was taken back into the classroom to S5 by S3. However, interviews confirmed the classroom was out of ratio at the time C5 left the classroom and there was no floater teacher at the time of the incident. In regard to a second incident, involving an unknown child being on the playground without supervision, confidential interviews revealed there were no witnesses to this incident and CCC was not aware of this occurring..

Based on confidential interviews and document review, the licensee is being cited for Teacher-Child Ratio, classroom Terrific Toddlers was not properly supervised based on the number of teachers and children present on or around April 6, 2022..

A copy of this report, LIC 809D, appeal rights and LIC811 were explained and provided to Director at time of visit. Also, Director was given Notice of Site Visit and reminded it must be posted for 30 days.

Director has agreed to provide training on Teacher-child ratios and child safety to all staff. Director will provide proof of training to all staff on or before POC due date. Director will provide LPA with a copy of the staff sign-in sheet and training curriculum via email.

Based on confidential interviews and document review, S5 was alone in the classroom with 14 toddlers while co-teacher went to the restroom which poses an immediate health, safety and/or personal rights risk to persons in care.
