25 | On 11/02/2022 at 11:16am, Licensing Program Analyst (LPA) Samantha Clenista conducted an unannounced Plan of Correction inspection for the purpose to ensure that the facility’s bodies of water (pools and jacuzzi’s) are appropriately inaccessible per regulation. Upon arrival, LPA met with Director, Lauren Miller, and proceeded to tour the facility. There were no children present during inspection.
On 09/23/2022, LPA conducted a case management site visit and inspected all licensed areas and the pool area that is on site. During that inspection, LPA observed there to be a total of three exits within the facility that led to a side yard. Part of that side yard had a walkway that had a climbable chain link fence dividing the preschool area and the area where all the bodies of water are located. This chain link fence measured at approximately 4ft. 10 inches from the ground, shy of the 5ft. requirement. LPA also observed there to be a self-latching gate within this chain link fence, however, did not self-latch at multiple distances.
On 11/01/2022, Assistant General Manager, Amy Alexander, contacted LPA stating that all corrections have been made to ensure that the fencing was fixed per regulation. LPA toured the facility and inspected the updated fencing. The chain link fence is now a rot iron fence that has vertical poles. Fencing height was measured and the gate that pulls away from the bodies of water was still observed to be shy of the required 5ft. The self-latching gate was operational, however did not self-latch when it was pulled away about 2 inches out. Deficiency that was issued on 09/23/2022 was observed not fully corrected yet. LPA consulted with Ms. Alexander via telephone during today's visit and Ms. Alexander stated that she will have someone come out and lift the fencing to meet 5ft. and tighten the spring so that it self-latches at all distances. Ms. Alexander stated that she will provide proof of correction to LPA no later than 11/09/2022.
No further deficiencies were observed during today’s inspection. Exit interview was conducted with Director. NOTICE OF SITE VISIT IS TO BE POSTED FOR 30 DAYS. LPA observed Director post notice of site visit. This report shall be kept available at the facility for public review for 3 years. |
