25 | On 10/31/23 at 8:30 AM Licensing Program Analyst (LPA) Adrian Mangina conducted a Case Management with Licensee Maria Louisa Ramirez Phillips, for the purpose of confirming that employee Genesis Meza Luna has been removed from the home. The San Diego North Child Care Regional Office received from the Care Provider Management Bureau a Confirmation of Removal for Meza Luna, Genesis on 10/19/23. During this visit today, LPA toured the home with the Licensee to confirm that Ms. Meza Luna is no longer employed in the home. Ms. Phillips states that Ms. Meza Luna was employed at the facility for only one day but she cannot remember which date exactly. Ms. Phillips stated that she fully understands that Meza Luna cannot be in the home. During the visit LPA observed that a treadmill in the dining are was set up as a bed and Licensee stated that children play games on the treadmill. Also during the visit, LPA ascertained that Meza Luna does not appear to be working in the home at this time.

LPA Mangina informed Licensee Maria Louisa Ramirez Phillips that this report dated 10/31/23 document one Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care.
Also, LPA Mangina informed the Licensee to provide a copy of this licensing report dated 10/31/23 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview conducted and report was reviewed with the Licensee, Maria Louise Ramirez Phillips. A notice of site visit was given to Licensee and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
