25 | On 5/29/2025, Licensing Program Analyst (LPA) Janette Romero made an unannounced visit to the facility to conduct a required annual inspection. LPA was greeted and granted entry by Caregiver, Grace Romero who was informed of the purpose of the visit. Grace reported they informed Licensee, Lolita Gatmaitan of LPA's visit at the facility. The facility has a fire clearance to serve five (5) non-ambulatory elderly residents. LPA was informed none of the current residents receive hospice services.
LPA toured the facility with Grace and observed the facility is made up of a one-story home with three (3) resident bedrooms, one (1) bathroom, a kitchen, dining area, living room, laundry room, and attached garage. Resident bedrooms had the required bedding, furniture, and lighting. The bathroom had a grab bar and non-skid mat in the shower. The hot water temperature in the bathroom measured at 116 degrees Fahrenheit. No bodies of water were observed on the premises. Indoor and outdoor pathways were free of obstruction. The facility met Departmental requirements for a two-day supply of perishable foods and seven-day supply of non-perishable foods. Medications are secured in a locked cabinet stored near the kitchen. The facility has additional incontinent supplies, towels, blankets and linens available for resident use. Grace tested one (1) of the smoke alarms/carbon monoxide detectors and LPA observed it to be operational. LPA also observed a charged fire extinguisher mounted near the kitchen. Grace has a criminal record clearance, is associated with the facility, and has a valid CPR/AED/first aid certificate expiring on 5/31/2026. Long Term Care Ombudsman's contact information, residents' personal rights, complaint procedures, facility sketch, and emergency disaster plan are posted on a hallway wall leading to the residents' bedrooms. LPA reviewed the facility's certificate of liability insurance, which expired on 3/24/2024. Grace reported they informed licensee who communicated they would search for a more current one at their home. During the visit, licensee/Grace were unable to produce a current copy of the facility's certificate of liability insurance. As a result, the facility will be cited. An exit interview was conducted and a copy of this report, LIC 809-D, and Appeal Rights were reviewed and provided to Grace.
