The facility is licensed to serve six (6) clients aged 18 to 59, all six (6) of whom must be ambulatory.
LPA, accompanied by Licensees Robert and Linda Rucker, toured the interior and exterior of the facility, and inspected each room. The facility was clean, sanitary, and in good repair. Medications, sharps, and toxins were all separately stored in locked cabinets and/or rooms, as required and inaccessible to clients in care. The facility's ambient temperature was 73 degrees Fahrenheit and the hot water on taps accessible to clients measured at 106.3 degrees Fahrenheit.
No pools or bodies of water are on the premises. Per Licensee, no firearms or ammunition are kept at the facility. Carbon monoxide detectors, emergency lighting, and the facility telephone were all working. Fire extinguisher(s) were present with current inspections. First aid kit(s) and manual were readily accessible. Required licensing postings were observed in visible areas of the facility. Staff and client records and Personal and Incidental (P&I) resources were reviewed, and were complete, accurate and up to date. The facility had current Liability Insurance, and active Surety Bonds, and their disaster drill was conducted on July 11, 2025.
