On 11/4/2024, Licensing Program Analyst (LPA) Janette Romero conducted an unannounced visit to the facility to conduct a required annual inspection. LPA met with Administrator, Alberto Laureano who was informed of the purpose of the visit. The facility is a licensed vendor of the San Diego Regional Center. During the visit, there was 16 clients and eight (8) care staff present.

LPA toured the entire facility’s interior and exterior with Administrator Laureano. The facility is made up of two (2) separate buildings with four (4) classrooms in one (1) building and a larger multipurpose room in the other building. The facility also has various staff offices, storage rooms, client bathrooms, a conference room and kitchen/dining room. LPA toured the kitchen and observed the kitchen area to be clean. Snacks are provided to clients during program hours. LPA observed snacks are stored in a safe and healthful manner. Restrooms were clean and had toilet paper, paper towels and soap readily available. The hot water temperature in two (2) bathrooms measured at 107- and 110-degrees Fahrenheit. Cleaning solutions/supplies and chemicals were secured in storage closets, inaccessible to clients. Client medication is secured in a hallway closet. LPA was informed there are currently seven (7) clients receiving routine medication assistance during program hours. LPA reviewed the Medication Administration Record for two (2) clients while Administrator Laureano reviewed their physical medications and no discrepancies were discovered. LPA reviewed random staff and client files. Staff files reviewed have a criminal background clearance and current CPR/first aid certification. Client files reviewed have an updated physician's report, Individual Program Plans and signed admission agreements. The facility’s last fire and earthquake drill was conducted on 10/6/2024. LPA was informed some clients are sensitive to the smoke alarm sound; therefore, the smoke alarms were not tested during today's visit. However, LPA reviewed the facility's fire inspection records, which notes the facility's last annual fire inspection, testing, and maintenance was conducted by ASI Fire Protection, Inc., on 10/9/2024 where the fire sprinkler system and smoke/heat detectors were tested and observed to be functioning properly. The facility has charged fire extinguishers mounted that were last serviced on 12/5/2023. During today’s visit, LPA did not observe any deficiencies. An exit interview was conducted and this report was reviewed and provided to Administrator Laureano.
