13 | Licensing Program Analyst (LPA) Iby Strong contacted Administrator Michelle Retzer via telephone to deliver findings. On November 17, 2021, Community Care Licensing (CCL) received a complaint alleging facility charged for a month of services not rendered and staff did not provide an admissions agreement. During the investigation, the Department attempted to collect facility records and conducted interviews. Interview with Administrator revealed that all residents and their responsible parties are issued a copy of the signed Admissions Agreement. Interview with the Administrator revealed that R1 was taken to the hospital and passed away while on hospice at their home, not the facility. Interview established that Administrator was not informed of resident’s passing until weeks after resulting in R1’s personal belonging remaining at the facility. Later, a refund was requested for R1’s final month, in which R1’s personal belongings remained at the facility. Administrator was then informed that the party responsible did not understand the Admissions Agreement and the parties responsible were provided with a copy of the agreement. According to Administrator, R1 was not eligible for a refund based on timeframes. There were no records available to corroborate the allegations. Based on the interviews, there is not a preponderance of evidence to prove alleged violations occurred, therefore the allegations are unsubstantiated. An exit interview was conducted and a copy of this report and Appeal and Licensee Rights (LIC 9058 03/22) were provided to the Administrator via email.
