Staff interviews concluded that the facility was performing a fire drill on the 27th of May when a client pulled the alarm and broke the handle. S1 stated that after the fire department inspected the device, they told S1 that a new handle system for the alarm would need to be replaced. S1 called an electrician to plug up the hole in the wall left by the broken handle. S1 stated they are currently in the process of having the alarm handle replaced.
LPA observations of the facility confirmed staff interviews. A metal plate the size of a light switch cover now obstructs the hole in the wall and no obvious disrepair or danger was observed.
Regarding the allegation, "Staff do not ensure all electrical are properly maintained", it was alleged that several parts of the home's electrical system were non operational or faulty.
Facility staff interviews confirmed that there is disrepair in the facility however these issues are directly related to the fire alarm issue, have been inspected by an electrician, and are to be repaired the following day of the LPA's visit, June 5th, 2026. S1 stated that the fire alarm system tripped the breakers in the back of the house and the electrician is due to check and label the systems to ensure compliance.
LPA observed one electrical outlet in the home that was in disrepair/scheduled to be repaired. One outlet had burn marks on the bottom plug opening however this was due to a response in the fire alarm system. The outlet is nonoperational at the moment as the breaker is off and there is no danger to staff or clients. The LPA observed no RV plugs attached to the facility though there was an RV parked in the back of the house. Additionally, the LPA observed two breakers in the back of the house were switched to the off position to prevent injury and to be inspected by the Electrician on the scheduled appointment.
Records review of a text conversation dated from May 28th to June 4th confirmed facility staff contacted and made an appointment for an electrician to replace the fire alarm handle and perform other electrical work on the house including outlet repair, wire testing, and breaker maintenance.
Based on interviews, direct LPA observations and records review, a preponderance of evidence does not exist to prove that the alleged violations occurred, therefore the allegations are UNSUBSTANTIATED. An exit interview was conducted with Licensee Sabrina Snyder, to whom a copy of this report and the Licensee/Appeal Rights (LIC9058 03/22) were provided. |
