13 | On February 16, 2023, at about 12:35 PM, Licensing Program Analyst (LPA) Daniel Pena conducted a complaint investigation visit to deliver findings regarding the above-mentioned allegations. LPA was greeted at the entrance by Executive Director, Sasha Hightower and granted entry after identifying himself and disclosing the reason for the visit.
It was alleged the facility's sewer system was not operating, causing an offensive odor.
This investigation consisted of facility tours, interviews with residents and staff and a review of facility and outside source records. Per facility records, at the time this complaint was filed, the daily census had decreased due to the COVID-19 pandemic. Records indicate there were 44 vacancies in the facility in July 2021. Interviews revealed that standing water in the vacant room's toilets and bathroom drains created a strong odor that some found offensive. The administration responded to the complaint and mobilized |
