Licensing Program Analyst (LPA) Javina George mailed this report to the ex-licensee’s last known mailing address via USPS certified mail as well as via the ex-licensee’s business email on record, to communicate the findings related to the above-mentioned allegation. The facility has been closed since 07/12/25.

On 08/18/25 Community Care Licensing received a complaint alleging that the licensee is not operating the facility. It was further alleged that on 7/1/24 the Riverside Regional Office was informed that there was a Change of Ownership (CHOW). This information could not be corroborated, however there was an application submitted to the Centralized Applications Bureau for a change of ownership on 07/12/24, due a result of a substantiated unlicensed complaint control number 18-AS-20240701144248, connected to this facility. LPA conducted a records review of Residential Lease or Month to Month Rental Agreement dated 06/15/24, which revealed Zayden Chen (Daybreak OPCA LLC) “tenant” enter into a lease agreement with GODM investment LLC/Solaris Capital Group Inc “Rental property Owner, RPO” with the

term ending on 06/15/28. The lease agreement is dated 07/03/24. A Further records review was conducted of the facility’s Administrative Organization form dated 05/20/2016, which indicates TWET LLC, as the Licensee, and the owner as Bin Shen and the term expiring in five (5) years in 2022.

Additionally, per the Statement of Information Limited Liability Company with the California Secretary of State for the date filed on 07/31/24 notes for TWET LLC as the entity and the manager to be Bin Shen, however Zayden Chen signed, as they were the one making the filing. There was an additional filing on 08/06/24 that lists the entity as TWET LLC and the Manager is listed as Zayden Chen. Per an email dated 08/20/25, Zayden explains that Bin Shen is the old shareholder of TWET, LLC. Per Mr. Chen he is 100% owner of TWET, LLC, as of 07/01/24.

Based on interviews and records review the allegation is unsubstantiated. A finding that the complaint is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation(s) occurred.

An exit interview was not conducted as the facility has been closed since 07/12/25. A copy of this report, was sent to the ex-licensee’s last known address via USPS certified mail and via the ex-licensee’s business email on record, due to the facility closure. Note there was no signature obtained as LPA did not meet with a facility representative, due to the facility being closed.
