During multiple department visits, LPA's toured the Memory Care Unit, common areas, and resident rooms. All rooms observed were clean, organized, free from odors, and stocked with toiletries, toilet paper, trash liners, clean bedding, towels, and additional linens. No soiled briefs or dirty laundry were observed, and bathrooms were clean and sanitary. Residents, including R1, were observed to be clean, appropriately dressed, and free from signs of neglect. Approximately 25 residents were observed eating lunch with caregiver and dining staff present to assist. Attempts to interview residents were unsuccessful due to dementia diagnoses that affected alertness and orientation. Residents were not determined to be credible witnesses but appeared clean and well cared for.
Department interviews with staff on multiple visits interviews did not corroborate the allegations. No staff interview revealed missed care, lack of supplies, inadequate staffing levels, or incomplete training. Record review did not produce documentation supporting any of the alleged issues. Department records review did not reveal missed incontinence care, lack of hygiene care, missing toiletries, insufficient staffing, or incomplete training. No incident report or evidence was provided regarding the reported black eye for R1. No photos or additional documentation were submitted by the Reporting Party.
Based on interviews, record review, observations, and lack of corroborating evidence, the Department did not find support for the allegation that staff failed to provide adequate care and supervision, including incontinence care, hygiene, toiletries, room cleanliness, staffing levels, or training. The allegation is determined to be UNSUBSTANTIATED.
An exit interview was conducted with the Executive Director, and a copy of this report and the Licensee/Appeal Rights (LIC 9058) were provided.
