Licensing Program Analyst (LPA) Dang Nguyen conducted an unannounced visit to commence a Complaint Investigation regarding the above allegation. LPA was welcomed by, identified himself to, and discussed the purpose of the visit with Licensees Raquel Preece and Derek Hampton.

The Complainant alleged that Licensee did help Client #1 (C1) maintain personal hygiene. [See LIC811 Confidential Names List for a description of C1.] CCLD’s investigation involved an unannounced facility tour/welfare check, review of pertinent care records, and interviews of relevant facility staff. LPA also met with and interviewed C1 on multiple days.

According to C1’s latest LIC602 Physician’s Report: C1 was diagnosed with intellectual disability but was also ambulatory, able to follow instructions, and able to communicate their needs. Their doctor determined that C1 was able to bathe themselves, able to dress/groom themselves, and able use the bathroom by themselves. It was also written that C1 was fully continent of bowel and bladder. According to C1’s San Diego Regional Center (SDRC) Individual Program Plan (IPP), C1 was “able to complete self-care tasks without assistance” but benefited from verbal prompts/reminders from staff.

Interviews of C1 and multiple staff unanimously showed: C1 was more of a “night-person” than a “morning-person.” C1 was indeed physically and cognitively capable of performing their own grooming, dressing, and bathing tasks, requiring only staff reminders, at most. Staff consistently provided these reminders to C1 (i.e., reminders to brush teeth in both the mornings and evenings, and reminders to bathe at night). In practice, C1 sometimes chose not to brush their teeth in the morning but brushed their teeth every night. C1 also consistently showered and changed into fresh clothes every night.

LPA interviewed C1 on both 02/21/2025 and 06/09/2025, respectively. On both dates: C1 had their eyeglasses on, was in clean clothing free of stains, and had a cleanly washed face. C1 had reasonably clean-looking teeth; LPA did not detect any bad breath. C1 had no detectable body odor. LPA also met and spoke with multiple of C1’s housemates/peers on these dates; LPA had no concerns regarding their personal hygiene.

Based on records and interviews, a preponderance of evidence does not exist to show that Licensee did help client maintain personal hygiene. This allegation is therefore Unsubstantiated, and no deficiency was cited for it.

Since Licensees Raquel Preece and Derek Hampton needed to attend an off-site appointment, they excused themselves part-way during LPA’s visit. LPA conducted the exit interview with DSP Brooke Preece-Hampton. A copy of this report and the Licensee/Appeal Rights (LIC9058 03/22) were provided to Licensee during today's visit.
