25 | Licensing Program Analyst, LPA Tiffany Holmes made an announced Pre-licensing and Component 111 visit to the facility. LPA met with Sonia Barron, Applicant/ Administrator. The Fire Department Inspector completed the inspection on 01/18/2022. The facility was approved for six (6) ambulatory only clients. The facility has the appropriate fire extinguisher, carbon monoxide detectors and fire alarms.
LPA toured the facility inside and out with the applicant. The facility is a double story home with five bedrooms, all five are client rooms. There are three bathrooms. The bathroom had paper towels and soap in them. Facility has the appropriate supply of hygiene items. Facility has first aid manual and all the required first aid items. The medications will be locked in a cabinet off the kitchen by the garage. The client and staff records will be locked in a closet in the same closet as the medications. There was a small supply of non-perishable foods. The facility has all the working appliances and the supplies to provide food service for the clients to operate. The home is clean and in good repair. There is plenty of space to conduct activities inside and out. There is a covered area in the backyard with table and chairs. Chemicals are locked in the garage in a locked cabinet. Knives are locked in the garage in a cabinet. The water temperature was checked and it was 116.2 degrees F. Discussed with the applicant continuing operation requirements, record keeping and physical plant compliance. The administrator certificate for Sonia Barron expires on 07/11/2023
The Pre-licensing and Component 111 was completed with Applicant. The facility is ready for licensure pending management approval. A copy of this report was emailed to applicant after the end of visit. |
